RESUMO

The objective of this study was to compare the clinical efficacy and cost-effectiveness of IL-17 inhibitors (SEC, IXE, NTK) in the treatment of adult patients with ankylosing spondylitis (AS) in the healthcare system of the Russian Federation. Materials and methods. The study is a sub-analysis of a previously published systematic review and network meta-analysis of the comparative efficacy of biologics in adult patients with AS in the Russian Federation. NNT values were calculated for BASDAI 50 and ASAS 20/40 after 16 weeks of therapy for all studied drugs. CpR was estimated for each biologic after 16 weeks and one year of therapy. Additionally, we carried out an assessment of the financial burden of the most cost-effective strategies for the treatment of AS. The use of NTK is characterized by an average of no more than three patients needed to treat to achieve one ASAS 20/40 or BASDAI 50 response, while on IXE and SEC no more than 4-5 patients need to be treated, depending on the estimated effectiveness criterion. According to CpR estimate, NTK is the most cost-effective IL-17 inhibitor for the treatment of AS, both after 16 weeks and after one year of therapy. The obtained results make it possible to compare the effectiveness of IL-17 inhibitors from a clinical and economic points of view and can be used both in decision making on treatment strategies for individual patients and at the population level when deciding on the reimbursement of drugs.

Indications for and organization of outpatient radiation and chemotherapy of cancer are discussed. Schemes of treatment for cancer of major sites are described. Possible contraindications for this were identified. Fund saving is another advantage of outpatient therapy.

RESUMO

The recent advances have made it possible for chemotherapy to be used for cancer control under the conditions of out-patient clinics. Within 5 years, 9,000 out-patients had received at the Center 10-day courses of chemotherapy which was free from toxic complications and produced a therapeutic effect similar to that observed in in-patients. The out-patient chemotherapy service saved 177,140 in-patient/days which is an equivalent of running a 500 bed specialized cancer hospital during 12 months.

RESUMO

Lipid-bound sialic acids were estimated in blood serum of 118 patients with tumors, of 13 donors and of 38 patients with non-tumoral diseases. Content of the sialic acids in blood serum of the patients with growing tumors constituted 23.1 +/- 1.1 mg/100 ml, which was 2-2.5-fold higher as compared with healthy donors (10.5 +/- 0.4 mg/100 ml) or with the patients with non-tumoral diseases (13.8 +/- 0.5 mg/100 ml). In the oncologic patients during the remission period the content of lipid-bound sialic acids in blood serum approached the normal level - 12.1 +/- 0.5 mg/100 ml. The highest level of the sialic acids was found in patients with lymphogranulomatosis II-IV step, with melanoma during the period of growth, with kidney tumor and with liver tissue primary tumor. Estimation of circulating lipid-bound sialic acids in the oncologic patients may serve as a test for evaluation of the tumor growth as well as of therapeutic treatment efficiency.

RESUMO

The concentration of the active principle of ftorafur in blood and urine varied depending on the activity of metabolic processes in a patient, size and sensitivity of tumor, etc. The concentrations increased in repeat injections of the substance. The efficacy was related with the intensity of formation of active components, but is not dependent on the substance level dtermined in biological fluids of the organism.
